Acute Effects on the Counts of Innate and Adaptive Immune Response Cells After 1 Month of Taoist Qigong Practice

Numerous physical as well as mental benefits have been classically ascribed to this traditional mind-body method which integrates slow body movements, breathing, and meditation. Albeit we have already reported an immunomodulatory action of qigong in other investigations, measures were then assessed 1 day after the qigong program ended. PURPOSE: The aim of the present study was to assess the acute effects of Taoist qigong practice on immune cell counts in healthy subjects 1 h after training. METHOD: Forty-three healthy subjects participated in the study of whom 25 were randomly allocated to the experimental group and 18 to the control group. The experimental subjects underwent daily qigong training for 1 month. Blood samples for the quantification of immune parameters (number and percentage of monocytes, neutrophils, eosinophils, total lymphocytes, B lymphocytes, and natural killer (NK) cells) were taken the day before the experiment commenced and 1 h after the last session of the training program ended. As statistical analysis, analysis of covariance (ANCOVA) was performed. RESULTS: Statistically significant differences were found between the experimental and control groups, with the experimental group showing higher values in the number (p = 0.006) and the percentage (p = 0.04) of B lymphocytes, as well as lower values in the percentage of NK cells (p = 0.05), as compared to control. CONCLUSION: This study demonstrates that Taoist qigong is able to exert acute immunomodulatory effects on components of both innate as well as adaptive immune response. Assessment of long-term cognitive impairment after off-pump coronary-artery bypass grafting and related risk factors

DESIGN: Prospective study. SETTING: Virgen de la Victoria University Hospital, Málaga, Spain. PARTICIPANTS: Participants were 36 patients undergoing off-pump coronary-artery bypass grafting. MEASUREMENTS: Changes in the neuropsychological test battery administered from before to after surgery (1, 6, and 12 months). Postoperative cognitive impairment was defined by a significant decrease. RESULTS: A significantly multidomain (attention-executive functions, P < .01; immediate and delayed memory, P < .001; and verbal fluency, P < .05) postoperative cognitive impairment was shown, being maximum at 6 months (more than 50% of patients) and still presented at 12 months (more than 30% of patients), but partially recovered. Related risk factors as smoking (P < .01), diabetes mellitus (P < .01), peripheral arteriopathy (P < .01), obesity (P < .05), lower hematocrit (P < .01), and hemoglobin (P < .05) levels and diastolic blood pressure (P < .05) were identified as predictors of cognitive impairment. Better New York Heart Association class (P < .01) and less severity of angina (P < .01) were associated with partial postoperative recovering. CONCLUSION: A multidomain long-term postoperative cognitive impairment and a partial neurocognitive recovering were detected after off-pump coronary-artery bypass grafting and were associated with several nonspecific surgery factors. These findings may be useful when counseling patients before surgery and suggest the importance of long-term neurocognitive evaluation. Bilateral acute retinal necrosis due to varicella zóster virus in an elderly patient

Polymerase chain reaction (PCR) of aqueous humor was positive for Varicella Zoster virus (VZV). He developed a Kyrieleis' vasculitis a month after the starting treatment, when the PCR analysis was negative. DISCUSSION: PCR is a quick and safe technique, with a high sensitivity and specificity of 97%, useful to diagnose and monitor the viral activity. The intervention must be urgent, due to the dramatically rapid evolution. Oral famciclovir oral is good alternative owing to its bioavailability. Clinical, virological and phylogenetic characterization of a multiresistant HIV-1 strain outbreak in naive patients in southern Spain

DOI 10.1093/jac/dkv332 Catálogo de biblioteca PubMed Idioma ENG Resumen BACKGROUND: We describe the characteristics of an HIV-1 strain with six viral reverse transcriptase mutations (D67N, T69N/D, V118I, V179D, T215S and K219Q), which we have called the Malaga strain. This strain was detected in treatment-naive patients from southern Spain. METHODS: The study was undertaken at the Virgen de la Victoria Hospital, Malaga, a reference centre for the study of HIV-1 genotype resistance in Andalusia (the 'Costa del Sol'), Spain. Genotypic resistance testing was done in an automated sequencer. Phylogenetic analysis was performed using a 630 bp region of the reverse transcriptase with the mutations mentioned. RESULTS: Between 2007 and 2014, we detected the Malaga strain in 30 treatment-naive patients. All were MSM, seen at five hospitals on the Costa del Sol. In all cases, the HIV-1 was subtype B with viral tropism R5. Phylogenetic analysis based on the reverse transcriptase sequence showed consistent grouping (with a bootstrap value of the common node of 100%) of the isolates that shared the mutation pattern mentioned. This strain has not been detected elsewhere or in previously treated patients. All of the patients treated with first-line combination ART responded. CONCLUSIONS: We report a cluster of an HIV-1 strain with multiple resistance mutations that was transmitted over a period of >8 years, affecting 30 naive patients from the same geographical area. Her visual acuity in that eye was 20/20. Recently, she came to our department with a sudden visual loss and metamorphopsis in her left eye. After performing funduscopy, angiography and OCT, she was diagnosed with choroidal neovascular membrane (CNVM) at lesion border, and started on antiangiogenic therapy. DISCUSSION: CHR-RPE, despite being a benign condition, may become complicated with severe visual impairment. Antiangiogenic therapy provides a good alternative to photodynamic therapy or laser photocoagulation for treatment of CNVM, avoiding adding iatrogenesis from these treatment to the complications associated with this pathology. Drug utilization pattern in children and off-label use of medicines in a pediatric intensive care unit

METHODS: A prospective observational study was conducted on a dynamic cohort of children admitted to an NPICU (N=81) in a tertiary hospital (Granada, Spain). All prescriptions were classified as off-label or unlicensed based on the summary of product characteristics (SPC). RESULTS: Of a total of 601 prescriptions, the patients received a mean of 7.4±6 drugs each. The most commonly prescribed drugs corresponded to classes J (anti-infectious, systemic use) N (nervous system) and C (cardiovascular). A little over one-half of the prescriptions were off-label (52%), usually due to dosages differing from the SPC recommendations (79%), followed by different indications (13.5%), age (5%) and administration route (2.5%). In this NPICU, unlicensed usage represented only 5% of all prescriptions. CONCLUSIONS: This study contributes data on prescription of this kind in a Spanish NPICU, revealing at least one off-label prescription in 89% of the children and at least one unlicensed use in 22.3%. These are high figures, but are to be expected given the inclusion of newborn infants and the critical care setting. Even though such usage follows clinical protocols, we underscore the dual need to base treatment on the best available evidence, and to upgrade the SPC accordingly. [Faecal calprotectin as an aid to the diagnosis of non-IgE mediated cow's milk protein allergy]

PATIENTS AND METHODS: A one year prospective study was conducted on 82 infants between 1 and 12 months of age in the Eastern area of Málaga-Axarquía, of whom 40 of them had been diagnosed with non-IgE-mediated CMA (with suggestive symptoms and positive response to milk withdrawal), 12 not diagnosed with CMA, and 30 of them were the control group. FCP was measured at three different times: time of diagnosis, and one and three months later. ANOVA for repeated measures, nominal logistic regression and ROC curves were prepared using the SPSS.20 package and Medcalc. RESULTS: Differences between diagnostic and control groups were assessed: there was a statistically significant relationship (p<.0001) between high FCP levels and infants suffering CMA, as well as the levels at time of diagnosis, 1 and 3 months (p <.001). A ROC curve was constructed between FCP levels and diagnosis of CMA, with 138 ug/g, with the best cut-off being with an area under the curve of 0.89. However, it is only 0.68 to predict a clinical response. CONCLUSIONS: FCP levels lower than 138ug/g could be useful to rule out non-IgE-mediated CMA diagnosis. The aim of this study was to analyze clinical and echocardiographic outcomes in a series of patients who received surgical treatment for severe tricuspid regurgitation and to compare outcomes according to the operative approach to valve repair or replacement. METHODS: Retrospective study in 119 consecutive patients with severe tricuspid regurgitation undergoing valve surgery between April 1996 and February 2010. RESULTS: A total of 61 ringless and 23 ring annuloplasties were performed and 11 bioprostheses and 24 mechanical prostheses were implanted. Perioperative mortality was 18.5% and was associated with age and cardiopulmonary bypass time. During clinical follow-up (median, 41 [interquartile range, 24-89] months), 2 reoperations were required in the ring annuloplasty and mechanical prosthesis groups; prosthetic thrombosis was diagnosed in 4 patients in the latter group. Total mortality after follow-up was 29.9% and was associated with age>70 years and extracorporeal circulation time. The emergence of new severe tricuspid regurgitation was associated with age and ringless annuloplasty (P=.04). CONCLUSIONS: Ringless repair was significantly associated with recurrence of severe tricuspid regurgitation. The use of mechanical prostheses was associated with a high rate of thrombosis. No significant differences in perioperative or total mortality were found between the different methods used for repair or valve replacement. "El origen de esta base de datos está en la década de los 90 cuando se empezó a gestar como instrumento de aprendizaje para el estudio radiológico en hospitales militares. Posteriormente se unirían otro tipo de imágenes y otro tipo de hospitales que han contribuido a ella. En 2001 se incluyeron también las immágenes de educación médica y de formación de enfermería.
Presenta un caso cada semana donde se pueden leer los principales datos y hacer una autoevaluación de los conocimientos
Las características principales de la base de datos son:
- La gran calidad de las imágenes
- La clasificación exahustiva por cada una de ellas (órganos, sistema, patología, datos del paciente, especialidad médica o quirurigca, etc)
- La mayoría de los casos, tanto los comunes como los raros, tiene un diagnóstico confirmado (patología, seguimiento clínico, etc)

The document is set out in different sections as regards the diagnosis and treatment of, overt and subclinical hypo- and hyperthyroidism, isolated hypothyroxinaemia and postpartum thyroiditis. It also justifies the implementation of universal screening for thyroid dysfunction in pregnancy, and provides practitioners who care for these patients with tool for rational decision making. The aim of this study was to analyze clinical and echocardiographic outcomes in a series of patients who received surgical treatment for severe tricuspid regurgitation and to compare outcomes according to the operative approach to valve repair or replacement. METHODS: Retrospective study in 119 consecutive patients with severe tricuspid regurgitation undergoing valve surgery between April 1996 and February 2010. RESULTS: A total of 61 ringless and 23 ring annuloplasties were performed and 11 bioprostheses and 24 mechanical prostheses were implanted. Perioperative mortality was 18.5% and was associated with age and cardiopulmonary bypass time. During clinical follow-up (median, 41 [interquartile range, 24-89] months), 2 reoperations were required in the ring annuloplasty and mechanical prosthesis groups; prosthetic thrombosis was diagnosed in 4 patients in the latter group. Total mortality after follow-up was 29.9% and was associated with age>70 years and extracorporeal circulation time. The emergence of new severe tricuspid regurgitation was associated with age and ringless annuloplasty (P=.04). CONCLUSIONS: Ringless repair was significantly associated with recurrence of severe tricuspid regurgitation. The use of mechanical prostheses was associated with a high rate of thrombosis. No significant differences in perioperative or total mortality were found between the different methods used for repair or valve replacement. The objective of this study was to identify differences between physicians' and T2DM patients' perceptions related to health status, patient-reported outcomes assessments, and T2DM management and treatment, in routine clinical practice in Spain. METHODS: This was an observational, cross-sectional study including 1,012 T2DM patients and 974 physicians from 47 and 52 Spanish provinces, respectively. An electronic structured self-administered questionnaire containing 17 questions was designed aiming to address both physicians' and patient's perceptions on overall T2DM health status and patient-reported outcomes. RESULTS: T2DM patients perceived a worse health status (40% reported having a "good" and 38% a "neither good nor bad" health status) compared with physicians' perceptions (77% thought patients had a "good" health status). Most patients answered being "satisfied" or "neither satisfied nor unsatisfied" with the given information, while physicians considered that patients were "satisfied" or "very satisfied" with the information for self-monitoring blood glucose and treatment administration. Fifty-seven percent of patients reported that medical recommendations were "important", while 58% of physicians considered it as "very important". Fifty-three percent of patients perceived that their current T2DM treatment suited their preferences "quite a lot", and this was lower than the proportion of physicians (69%) that believed this for their patients. Additionally, a lower percentage of patients (53%) than physicians (79%) believed that their treatment improved their health-related quality of life "quite a lot". PATIENTS AND METHODS: A one year prospective study was conducted on 82 infants between 1 and 12 months of age in the Eastern area of Málaga-Axarquía, of whom 40 of them had been diagnosed with non-IgE-mediated CMA (with suggestive symptoms and positive response to milk withdrawal), 12 not diagnosed with CMA, and 30 of them were the control group. FCP was measured at three different times: time of diagnosis, and one and three months later. ANOVA for repeated measures, nominal logistic regression and ROC curves were prepared using the SPSS.20 package and Medcalc. RESULTS: Differences between diagnostic and control groups were assessed: there was a statistically significant relationship (p<.0001) between high FCP levels and infants suffering CMA, as well as the levels at time of diagnosis, 1 and 3 months (p <.001). A ROC curve was constructed between FCP levels and diagnosis of CMA, with 138 ug/g, with the best cut-off being with an area under the curve of 0.89. However, it is only 0.68 to predict a clinical response. CONCLUSIONS: FCP levels lower than 138ug/g could be useful to rule out non-IgE-mediated CMA diagnosis. Her visual acuity in that eye was 20/20. Recently, she came to our department with a sudden visual loss and metamorphopsis in her left eye. After performing funduscopy, angiography and OCT, she was diagnosed with choroidal neovascular membrane (CNVM) at lesion border, and started on antiangiogenic therapy. DISCUSSION: CHR-RPE, despite being a benign condition, may become complicated with severe visual impairment. Antiangiogenic therapy provides a good alternative to photodynamic therapy or laser photocoagulation for treatment of CNVM, avoiding adding iatrogenesis from these treatment to the complications associated with this pathology. Polymerase chain reaction (PCR) of aqueous humor was positive for Varicella Zoster virus (VZV). He developed a Kyrieleis' vasculitis a month after the starting treatment, when the PCR analysis was negative. DISCUSSION: PCR is a quick and safe technique, with a high sensitivity and specificity of 97%, useful to diagnose and monitor the viral activity. The intervention must be urgent, due to the dramatically rapid evolution. Oral famciclovir oral is good alternative owing to its bioavailability. Assessment of long-term cognitive impairment after off-pump coronary-artery bypass grafting and related risk factors

DESIGN: Prospective study. SETTING: Virgen de la Victoria University Hospital, Málaga, Spain. PARTICIPANTS: Participants were 36 patients undergoing off-pump coronary-artery bypass grafting. MEASUREMENTS: Changes in the neuropsychological test battery administered from before to after surgery (1, 6, and 12 months). Postoperative cognitive impairment was defined by a significant decrease. RESULTS: A significantly multidomain (attention-executive functions, P < .01; immediate and delayed memory, P < .001; and verbal fluency, P < .05) postoperative cognitive impairment was shown, being maximum at 6 months (more than 50% of patients) and still presented at 12 months (more than 30% of patients), but partially recovered. Related risk factors as smoking (P < .01), diabetes mellitus (P < .01), peripheral arteriopathy (P < .01), obesity (P < .05), lower hematocrit (P < .01), and hemoglobin (P < .05) levels and diastolic blood pressure (P < .05) were identified as predictors of cognitive impairment. Better New York Heart Association class (P < .01) and less severity of angina (P < .01) were associated with partial postoperative recovering. CONCLUSION: A multidomain long-term postoperative cognitive impairment and a partial neurocognitive recovering were detected after off-pump coronary-artery bypass grafting and were associated with several nonspecific surgery factors. These findings may be useful when counseling patients before surgery and suggest the importance of long-term neurocognitive evaluation. Numerous physical as well as mental benefits have been classically ascribed to this traditional mind-body method which integrates slow body movements, breathing, and meditation. Albeit we have already reported an immunomodulatory action of qigong in other investigations, measures were then assessed 1 day after the qigong program ended. PURPOSE: The aim of the present study was to assess the acute effects of Taoist qigong practice on immune cell counts in healthy subjects 1 h after training. METHOD: Forty-three healthy subjects participated in the study of whom 25 were randomly allocated to the experimental group and 18 to the control group. The experimental subjects underwent daily qigong training for 1 month. Blood samples for the quantification of immune parameters (number and percentage of monocytes, neutrophils, eosinophils, total lymphocytes, B lymphocytes, and natural killer (NK) cells) were taken the day before the experiment commenced and 1 h after the last session of the training program ended. As statistical analysis, analysis of covariance (ANCOVA) was performed. RESULTS: Statistically significant differences were found between the experimental and control groups, with the experimental group showing higher values in the number (p = 0.006) and the percentage (p = 0.04) of B lymphocytes, as well as lower values in the percentage of NK cells (p = 0.05), as compared to control. CONCLUSION: This study demonstrates that Taoist qigong is able to exert acute immunomodulatory effects on components of both innate as well as adaptive immune response. Nuevo doctor en la UGC de Laboratorio del Área de gestión Sanitaria Este de Málaga-Axarquía.
El pasado 9 de febrero en el salón de Grados de la facultad de Psicología de la Universidad de Málaga, defendió el doctorando D. Francisco Rodríguez Peña la tesis titulada " Yoga y Meditación Mindfulness: Psiconeuroinmunoendocrinología e implicaciones clínicas".

Resumen:
Las disciplinas psicosomáticas orientales, como el yoga y la meditación mindfulness, han sido un instrumento muy utilizado en la cultura Oriental , desde tiempos remotos, por los numerosos beneficios para la salud atribuidos a su práctica. El yoga proviene de la antigua cultura india y su práctica implica el desarrollo físico, mental y espiritual. La meditación mindfulness proviene de las prácticas budistas Vipassana y Zen y su práctica implica concentrarse en el momento presente de forma activa y plena, siendo totalmente conscientes de lo que estamos haciendo, pensando o sintiendo. El objetivo del presente estudio fue investigar los efectos de la práctica de yoga y meditación mindfulness sobre parámetros psicobiológicos, tanto en practicantes experimentados en yoga, como en pacientes con síntomas de ansiedad y depresión sometidos a un breve programa de meditación mindfulness.
Los resultados en el estudio de yoga mostraron, en el grupo experimental, puntuaciones más bajas en el cuestionario de calidad subjetiva de sueño (PSQI) y niveles de cortisol en sangre más altos que en el grupo control. En los estudios de meditación mindfulness, los resultados en el post-test revelaron una mejora en la puntuación del cuestionario Mental Health 5 (MH-5), mientras que se elevaron los niveles de DHEA-S y ACTH, y los niveles de PTH disminuyeron. Además, demostraron un aumento significativo en las concentraciones de IgG, IgM, C3 y C4, así como un descenso en el recuento de monocitos. Por tanto, la práctica habitual de yoga se asocia con importantes cambios psicobiológicos, incluyendo una mejor calidad del sueño, así como una acción moduladora sobre los niveles de cortisol. Al mismo tiempo, nuestros resultados sugieren que la meditación mindfulness puede ejercer una modulación psicobiológica significativa, a través de la mejora del bienestar psicológico, así como de la regulación en los niveles de DHEA-S, ACTH y PTH. Asimismo, parece ejercer una acción inmunomoduladora en pacientes con ansiedad y depresión. Por lo tanto, podemos concluir que las prácticas psicosomáticas orientales, como el yoga y la meditación mindfulness, pueden ejercen un efecto modulador entre los sistemas nervioso, endocrino e inmunitario, y muestran un interesante y potencial uso en la práctica clínica.

11 de abril. Día Mundial del Parkinson
La Organización Mundial de la Salud declaró en 1997 que el día 11 de abril sería el Día Mundial del Párkinson, coincidiendo con el aniversario del neurólogo británico James Parkinson, quien en 1817 descubrió lo que en aquel tiempo él definió como "parálisis agitante" y que hoy conocemos como "enfermedad de Párkinson".La enfermedad de Párkinson (EP), un trastorno neurológico crónico, neurodegenerativo e invalidante, afecta en la actualidad a más de 150.000 personas en España; según estimaciones de los expertos, 1 de cada 5 pacientes son menores de 50 años. Quienes padecen párkinson requieren una atención integral socio-sanitaria que abarca desde terapias rehabilitadoras hasta un tratamiento farmacológico atendiendo las necesidades de cada paciente.
Desde entonces, asociaciones de Párkinson de todo el mundo conmemoran ese día con la celebración de actos públicos de interés general que pretenden difundir la realidad del Párkinson y sensibilizar a la sociedad en general.
La Federación Española de Párkinson (FEP) ha escogido en 2016 el lema #DíaDeMuchasPersonas, el DíaMundial de todas aquellas personas que conviven con la enfermedad (afectados, terapeutas, voluntarios, familiares, etc.).

12 de abril día nacional de la Atención Primaria
Según se desprende de la Declaración de Alma-Ata, la Atención Primaria “es la asistencia sanitaria esencial basada en métodos y tecnologías prácticos, científicamente fundados y socialmente aceptables, puesta al alcance de todos los individuos y familias de la comunidad mediante su plena participación y a un costo que la comunidad y el país puedan soportar, en todas y cada una de las etapas de su desarrollo con un espíritu de autorresponsabilidad y autodeterminación. La atención primaria forma parte integrante tanto del sistema nacional de salud, del que constituye la función central y el núcleo principal, como del desarrollo social y económico global de la comunidad.”
El Día Nacional de la Atención Primaria se celebra cada 12 de abril desde 2011 a instancias del Foro de Médicos deAtención Primaria que agrupa desde 2010 a las más importantes sociedades de médicos de atención primaria de España: semFYC, SEMG, SEMERGEN, AEPap, SEPEAP, OMC,CEEM y CESM.
El lema elegido para este año es 30 años cuidando. A nivel internacional, sin embargo, la WONCA (World Organization of Family Doctors) estableció el 19 de mayo en 2010 como el Global Family Doctor Day. 17 de abril. Dia mundial de la hemofilia.
La hemofilia es una enfermedad genética que impide la correcta coagulación de la sangre. Por ello quienes padecen esta enfermedad pueden sangrar más tiempo de lo normal o sufrir hemorragias espontáneas. Este trastorno de la coagulación, debido a una deficiencia del factor VIII (hemofilia A) o IX (hemofilia B), afecta a 1 de cada 1.000 personas en el mundo y la mayoría de ellos no ha recibido ni diagnóstico ni tratamiento.
Cada 17 de abril se celebra el Día de la Hemofilia para incrementar la concienciación sobre esta patología y el apoyo a quienes viven con este trastorno hematológico. Este día fue establecido desde 1989 en honor de Franz Schnabel, fundador de la Federación Mundial de Hemofilia.

28 de abril día de la Seguridad y Salud en el Trabajo
Promovido por Naciones Unidas y organizado por la OIT (Organización Internacional del Trabajo), cada 28 de abril, desde 2003, se celebra el Día Mundial de la Seguridad y Salud en el Trabajo (SafeDay). Desde 1991, estaba instituida esta fecha, por la canadiense Workers Mourning Day Act, como el Día del duelo por los fallecidos y heridos en el trabajo, posteriormente la conmemoración se amplió a todo el planeta. El objetivo de esta jornada es la prevención, para ayudar a reducir las muertes y lesiones relacionadas con el lugar de trabajo en el mundo, así como el recuerdo a las víctimas de accidentes laborales.El eslogan para la campaña de este año es Estrés en el trabajo: un reto colectivo. El estrés relacionado con el trabajo es actualmente reconocido como problema global que afecta a todos los países, todas las profesiones y todos los trabajadores, tanto en los países desarrollados como en desarrollo. En este complejo contexto, el lugar de trabajo es una fuente importante de riesgos psicosociales y al mismo tiempo el lugar idóneo para tratarlos y proteger la salud y el bienestar de los trabajadores.
